Update Your Listing

Please use this form to make changes or update your current listing in the Midwest Holistic Directory.

All updates must be received by January 25, 2009 to be included in the printed directory.
 
For the convenience of our listing holders we have changed the deadline to submit listing updates in the printed
directory from December 15, 2008 to January 25, 2009. This will not change again and no exceptions can be made
as the printed directory will be released January 31, 2009.

Updates made after January 25, 2009 will reflect in the online search facility only.

This form cannot be used to submit a new listing.
